What does the Community Asthma Partnership at Wolfson (CAP-W) do?

CAP-W is a partnership among some of Jacksonville’s leading asthma experts and serves as a resource for all things asthma on the First Coast. We offer programs and services to help families, educators, early childhood development professionals, and health providers to better understand the disease, manage it in all aspects of life, and improve access to quality asthma care throughout the community.

Serving all areas of Northeast Florida, CAP-W is working to:

  • Address the community’s unmet asthma needs, especially among those who do not have access to quality health care
  • Collaborate with health care organizations throughout our community to expand outreach to as many children and families as possible
  • Offer practical programs to teach the community about asthma in convenient locations including:
    • Schools
    • Health Care Settings
    • Faith-Based Organizations
    • Homes visits pending referral to Family Support Services ( case by case basis)
  • Share the nationally acclaimed Easy Breathing® program with pediatricians and family doctors to help them identify, treat, and manage asthma as early as possible
